First visit form

During your first appointment, you will need to fill out a form giving us all the relevant information about your state of health.

Terms of payment

It will be our pleasure to handle your claim with your dental insurance company via e-mail, for claims or payments.

Your child's first visit

We advise you to make an appointment with your child around the age of two or 6 months after the appearance of their first teeth.
